Job Summary:

  • The Program Coordinator will oversee the Diabetic Program with planning, implementation, and evaluation
  • The Program Coordinator will collaborate with the Nutrition Service Director, Physician Advisor and members of the diabetes team
  • The Diabetes Educator is responsible for utilizing the nursing process to provide individualized patient care by adhering to the essential functions identified in the Registered Nurse position description.
  • In addition to the Registered Nurse functions, the Diabetic Educator works with the diabetes education team to provide information to inpatients, outpatients and their family or support persons regarding the management of diabetes by individual and group instruction
  • The Diabetic Educator schedules outpatient appointments, provides documentation and bills for services provided

Requirements: 

  • Active Licensed Iowa Registered Nurse or compact state
  • Certification as a Diabetes Educator or eligible for certification through the National Certification Board of Diabetes Educators is required or willing to obtain upon hire
  • Coordinator will have continuing education which includes classes that relate to program management strategies, including planning, implementation and evaluation of all aspects of DSME Program
  • Continuing education shall also include training the in the care of persons with chronic disease

Schedule:

  • Mon-Fri
  • 72 or 80 hours a pay period - your choice!
  • 8a-4:30p
  • Schedule may fluctuate based on the needs of the department
